About the role:

We are looking for a qualified Nurse to work as an Admission and Discharge Coordinator Nurse in the Hospice based multi-disciplinary Specialist Palliative Care In Patient Unit. The purpose of the role is to ensure a smooth transition for patients as they arrive and when they are discharged from the In-Patient Unit. You will work alongside the Multi-Disciplinary Team to provide holistic, person-centred care and support.

You ll feel comfortable liaising with a variety of people in a compassionate and considered manner.
