Hybrid flexible, Consists of a Team Manager, 2 Senior Social Workers and a number of social work and support coordinator roles. As part of the Team, they also have a Transition Team with a Senior Transition coordinator and social workers. Wanting an experienced Social worker with Transitions and LD You will be focusing on a number of Care Act assessments of people living in a local setting which has both supported living and residential care placements.
